Located within the new facility, the Perham Health Pharmacy is a full service retail pharmacy offering complete pharmaceutical care, deliveries, and mail-out prescriptions through numerous insurance programs. This pharmacy serves patients from all physician groups in both Perham and the surrounding communities.
The pharmacy also offers MTM-- Medication Therapy Management. With this service, a pharmacist will sit down with you, go over any concerns you may be having, review your lab results, and look for ways to help improve your treatment plan. This individualized attention helps to insure you receive the best treatment plan.
MTM is offered to qualifying patients through their insurance company, and is also available to everyone on a cash basis. The service also results in a care plan, tailored to individual needs, that is sent home with patients. Examples of suggestions made on care plans could include consuming less salt or lowering your blood sugar.
An initial visit is about an hour, with follow-up visits also available.
